These controls affect only SurveyClear demonstration entries saved in this browser. They do not change server records, accounts, audit history, or other devices.
Export a backup first. Reset requires typing a confirmation phrase and does not run automatically.
Fictional information only. Reset cannot be undone unless you retain your downloaded backup. Refresh the page after resetting.
Shared facility workspace snapshot
Read-only developer view of PIP, survey POC, and QAPI records saved in the local shared database. Requires an active demonstration account session. This does not synchronize the clinical editors.
QUALITY IMPROVEMENT WORKSPACE · CONCERN INTAKE
Clinical Improvement Review
Document a fictional concern and choose an appropriate next step. A review is not automatically a PIP or survey Plan of Correction.
Saved clinical concern reviews
These are separate browser-local intake records, not formal PIPs, POCs, or QAPI reviews. Shared publishing and importing are optional actions below. Browser-local entries do not automatically synchronize.
Shared Clinical Reviews · Developer Preview
Optional, explicit publish/import for fictional records. Browser drafts do not automatically sync. A linked browser-local PIP is not a shared PIP.
SURVEYCLEAR TOOL GUIDE
Know what each tool is for
Operational quick references for facility leaders. These cards describe intended workflows; features still under development are identified.
Fictional demonstration only. This guide is educational and does not replace facility policy or professional judgment.
Requires the local Python server and a fictional demonstration account. Browser-local tools below remain separate.
Shared facility snapshot
Optional, read-only database view for signed-in demonstration accounts. These counts are separate from the browser-local dashboard; no automatic synchronization is implied.

QUALITY IMPROVEMENT WORKSPACE
Turn Clinical Concerns Into Measurable Improvement
Identify opportunities, develop corrective actions, monitor compliance, and document sustained improvement—all in one place.
Demonstration only · Fictional data · Do not enter protected health information (PHI).
Next steps
Review your saved improvement plans or create a new one.
Quick actions
Your entries are saved locally on this device for this demonstration.
SurveyClear Insights · Curated demonstration index
Regulatory Insights
Search F-tags and clinical topics. Review possible regulatory connections before using them in an improvement project.
Federal CMS · Florida-first roadmap
Verification required: This is a limited, manually curated reference index—not live CMS research. Descriptions are simplified, and regulations, survey guidance, applicability, and effective dates must be checked against current official sources. F-tags are not a finding of noncompliance.
Official reference starting points
These external links open authoritative source sites; verify the current version and applicable survey guidance before relying on them.
Turn a concern into a four-step improvement plan, refine it for your facility, and keep review notes in one place.
Prototype disclosure: Guided drafts and the optional suggestion buttons use local rule-based templates, not a connected AI model. Real AI will be optional at the organization level when a secure backend is available. Comments and plans are saved only in this browser, not shared with regional leadership. Do not enter PHI or sensitive facility information.
1 · Describe the concern
Shared PIP records · Developer preview
Use a signed-in demonstration account to publish or retrieve fictional PIPs from this facility's database. Your current PIP stays browser-local until you explicitly publish it. Never enter resident information.
Open through the local Python server to connect. Opening the HTML file alone will not activate shared records.
Live AI drafting · local developer server (not production-ready)
Optional. The existing local suggestion buttons still work without an AI connection. Use fictional or de-identified concerns only.
Suggested PIP — review before applying
Prototype uses local drafting templates, not a live AI model. Suggested root causes are questions to investigate, not established findings. Review facility policies before accepting.
Four-step corrective-action format. Step 1: immediate safety; Step 2: other residents at risk; Step 3: systemic prevention; Step 4: monitoring. Existing saved PIPs from older versions: their Step 1 and Step 2 text may still contain the former problem statement and root cause analysis. Review and manually reorganize those records before approving or exporting; SurveyClear will not silently rewrite them.
2 · Edit and verify the four steps
AI-ready structure. Edit every field to reflect actual evidence and company policy.
Not yet approved
3 · Action accountability
Assign owners, due dates and evidence references. Completing an action does not establish sustained compliance.
Local prototype
4 · PIP Insights
Track actual audit compliance separately from completion of assigned work.
Example-ready metrics
Audit data is entered manually and saved only in this browser. Results are not independently verified. The first audit is a baseline candidate, not proof of sustained improvement.
5 · Leadership review & follow-up
Record questions and recommendations against this PIP. In this demo, comments are local notes only; they are not sent to other users.
Clinical Improvement Builder
Clinical Improvement Builder
Start with any clinical or operational concern, or select a known F-tag. The post-fall project is an optional fictional example.
SurveyClear Corrective Action
Action Tracker
Assign tasks, track deadlines, and update status. Sample tasks are fictional and require validation.
Assigned corrective actions
Task status is a demonstration record, not proof that clinical actions were completed. Never enter resident identifiers.
SurveyClear Monitoring
Clinical Improvement Monitoring
Illustrative checklist for one mock review. Actual audits should be documented by event/date and based on verified evidence.

Dated audit log
Enter de-identified aggregate counts from an actual review, or use fictional demonstration numbers. Each entry is saved on this device.
The checklist above is a task demonstration, not an audit score. The dated log below calculates rates only from entered records; entries are not independently verified and should never be treated as proof of compliance.
SurveyClear Academy
Staff Education & Competency
A draft staff education outline aligned with the selected improvement concern.
Education: Immediate response and complete post-fall follow-up
Audience: Nursing staff and relevant interdisciplinary team members
Immediately assess the resident, ensure safety, and initiate emergency response as clinically indicated.
Notify the appropriate clinician and representatives according to orders, policy, and applicable requirements.
Record factual findings, timing, relevant circumstances, and assessments in the medical record.
Gather timely, objective statements from relevant staff; do not coach or prefill statements.
Investigate contributing factors and implement individualized interventions.
Update the care plan and communicate interventions across shifts and disciplines.
Escalate potential abuse, neglect, or other reportable incidents through the separate applicable reporting process.
Knowledge check
1. Is a fall investigation complete solely because an incident report was entered? No; follow-up must reflect the facts, investigation, interventions, and applicable policy.
2. Should staff statements be objective and collected promptly? Yes.
3. Should care plan changes reflect the resident's individualized risk factors? Yes.
Educational draft only. A clinical leader must review local policy, current regulations, reporting requirements, and the appropriateness of each step before implementation.
